Foundation Location 

The Foundation Office is located at 819 Summit Ave, Oconomowoc in the Oconomowoc Dental Center (Dr. Michael's dental office). 

Foundation Hours 

Monday through Friday – 8 AM to 5 PM

However, there are times during the day where the staff is at meetings and the office is closed.  Please call before you stop in to ensure we will be available.
