Honor & Memorial Gifts: Lasting Tributes to Loved Ones

Honor and memorial gifts are a wonderful way to show your respect and love for someone special.  Many individuals make an honor gift in celebration of a birthday, anniversary or special event in their lives. 

Memorial and honor gifts can be sent to the Oconomowoc Memorial Hospital and will be directed to the OMH Foundation Annual Fund for Greatest Need unless instructed otherwise.  Donors can choose to make any gift to any campaign or project  a tribute git. Wherever your gift is directed, be assured your gifts will be used to help Oconomowoc Memorial Hospital acquire new technology, fund community health programs and improve the quality of health care in the Lake Country Area.

If you are having an event,  memorial or church service and would like honor/memorial gift envelopes, please call the Foundation Office at 262-569-0222. Honor/memorial envelopes are also available at the Foundation office in the Oconomowoc Dental Center or in the main lobby area or Regional Cancer Center lobby at Oconomowoc Memorial Hospital.

You may include honor/memorial details on any gift you make online. But if you choose to mail your gift directly to us, please include the name of the individual being recognized along with the name and address of the individual, family or friend you would like notified about the gift.  All gift amounts are held in confidence.
