Transportation Services Guidelines

ProHealth Care (PHC) is committed to providing quality transportation services that are safe and accessible. Our specialized transportation services are for patients/customers who find it difficult or impossible to arrange for transportation to and from their medical appointments at PHC owned and staffed facilities. The PHC facility must be within our approved service area. Without this service, many patients would miss appointments, which can delay diagnosis and treatment. No other hospital or health care system in the area provides this level of service.

Keep this information in mind when planning to use ProHealth Care's transportation services.

Charge Information

  • You will be billed $7.50 per one-way trip ($15 round trip).
  • At the end of each month, you will receive an itemized invoice for transportation services provided within that specific month mailed to your home address. 
  • PHC drivers will not collect any cash or checks. 
  • If you require more than 16 visits to a ProHealth Care facility within the same month, you will be charged a maximum total of $120 (equivalent to 16 one-way trips).
  • Appointments not cancelled at least 24 hours prior to the appointment date will be assessed a $7.50 one-way trip charge.

Availability and Scheduling

  • Office hours are Monday through Friday 6:00 a.m. to 6:00 p.m.  Services are available for appointment times between 7:30 a.m. and 3:30 p.m.
  • Transportation services are available only in our approved service area (map.pdf).
  • All requests for transportation must be made 48 hours (2 working days) in advance through your care-provider or central scheduling personnel; otherwise transportation cannot be guaranteed.
  • Only patients/customers requiring non-emergent care/treatment should use this service.
  • Before scheduling transportation services, your care-provider or central scheduling personnel will determine if you qualify for transportation services by asking questions, such as whether you have any other means of getting to/from your medical appointments.
  • A transportation team member will telephone each patient/customer to confirm the initial appointment time and date.
  • Patients/customers must be ready at least one hour before their scheduled appointment times to ensure that on-time arrival and departure times for groups can be met.
  • If you no longer require transportation assistance, advise your care-provider that they should cancel transportation - or call (262) 928-7618 - at least 24 hours prior to the appointment date to avoid a $7.50 one-way trip charge.
  • If patient/customer is not available for scheduled pick-up and did not cancel transportation, it will be considered a "No Show".  After three "No-Shows", future transportation services may be denied.

Logistics

  • We are not equipped to transport patients/customers requiring IV support or ventilators. However, self-administered portable oxygen is acceptable.
  • Scooters, some power wheelchairs, lightweight wheelchairs and recliners cannot be safely secured for transport.
  • Patients/customers will be grouped by their geographic areas for pick-up and drop-off. Wait times may occur.
  • All patients/customers must wear seat and shoulder belts or be secured in the appropriate van seat.
  • Children under the age of 8 years old who weigh less than 80 pounds or are less than 57 inches tall will be transported in their own approved child safety or booster seat to comply with WI state statutes.
  • Children under the age of 13 years old must be accompanied by a parent/guardian.
  • No smoking is allowed in PHC vehicles or within 20 feet of a PHC vehicle.
  • No stops will be made for personal business or errands.
