At Oconomowoc Memorial Hospital, we know that patients' families and personal support networks play a key role in enriching the health and well-being of our patients and community.

That's why we've implemented a new, innovative care model that puts patients and families at the center of care in new and exciting ways. We're looking at everything from facility design and furniture to visiting hours and patient education from the patient's perspective. We know there's always more we can do to make sure our patients experience the best possible outcomes.

Partnership with Patients is a model of health care delivery that shapes policies, programs, facility design and staff day-to-day interactions with each other and patients and families. The model leads to better health outcomes and more appropriate allocation of resources -- all of which ultimately lead to greater patient and family satisfaction.

In the patient- and family-centered care model, health care providers recognize the value and critical role that families play in ensuring the health and well-being of all patients and family members. Health care providers in this model focus on the emotional, social and developmental support that are integral to quality health care delivery, and they promote the health and well-being of patients and families by restoring dignity and control to them.
