You may contact a patient relations representative at (262) 928-4784 or complete and submit the form below. A patient relations representative will respond within two to three business days.
Patient Relations will clarify the time frames necessary to investigate. You will receive a phone call and or a written response with the actions taken.
Complaints regarding quality of care or premature discharge will be reviewed by the responsible leaders on a timely basis. If your concern cannot be resolved through ProHealth Care's complaint process, you may contact The Joint Commission through their website or e-mail firstname.lastname@example.org.
