Advance Care Planning

Advance care planning enables individuals to make informed choices about their medical preferences in advance of an unforeseen event. ProHealth Care professionals can assist you in documenting the type of health care you would want to receive if you could not make your own decisions. With advance care planning, you can officially designate a health care agent to speak for you if you are unable to speak for yourself. 


ProHealth Care recognizes three forms of advance directives

Advance directives are legal documents written before you become terminally ill or incapacitated. The documents state your choices about treatments, or designate someone to make those choices for you. Through advance directives, you can make legally valid decisions about your future medical treatment before you are not able to do so for yourself.
