Senior Health Services
ProHealth Care’s senior health services offer a comprehensive memory loss assessment clinic designed to meet the needs of older adults and their caregivers. A multi-disciplinary team of geriatric-focused providers including a nurse practitioner, neuropsychologist, and psychiatrist will meet with the patient and family in one appointment to develop an individualized plan of care. The plan of care will include treatment recommendations, a review of ProHealth support services and community resources, and education for the patient and family.
- Daily living activities, such as bathing/dressing
- Mobility and home care needs
- Memory loss
- Mood changes
- Driving and safety concerns
- Nutrition and exercise
- Variety of psychosocial or behavioral issues
Testing and evaluation services are covered by Medicare Part B and most supplemental insurances.
The senior health assessment clinic is conveniently located in Suite 508 of the Professional Office Building adjacent to ProHealth Waukesha Memorial Hospital.
The senior health assessment clinic is open Monday through Friday 8 a.m. to 4:30 p.m.
