Because older adults have different health care concerns and needs, the Senior Health Center at Waukesha Memorial Hospital offers an Evaluation Service. In one appointment, a team of medical experts who specialize in caring for seniors will assess various health concerns, including, but not limited to:

  • Daily living activities, such as bathing/dressing
  • Mobility and home care needs
  • Medication
  • Memory loss
  • Depression
  • Nutrition and exercise
  • Vision and hearing
  • Variety of psychosocial or behavioral issues

Following the evaluation, a case manager from the Senior Health Center is available for up to 6 months. The case manager provides education, support and coordination of community resources for the patient and care provider through regularly scheduled phone contacts and home visits.

Cost Information

Testing and evaluation services are covered by Medicare Part B and may be covered by supplemental insurance; however, some additional testing or services may not be covered. There is an enrollment fee for outreach case management, which covers the initial home visit and development of your care plan.

The Senior Health Center is conveniently located in Suite 508 of the Professional Office Building, which is adjacent to Waukesha Memorial Hospital.

Our hours of service

The Senior Health Center is open Monday-Thursday 8:00 a.m.-4:30 p.m., and Friday 8:00 a.m.-12:00 noon.
