ProHealth Home Care & Hospice is one of Wisconsin's most comprehensive sources of hospice and home care. We provide a seamless continuum of care for people of all ages. Inpatient hospice care is offered at AngelsGrace Hospice, a beautiful, 15-bed residential hospice nestled on eight serene acres in Oconomowoc. ProHealth Home Care & Hospice also offers quality in-home hospice care and in-home skilled nursing care. We are part of the ProHealth Care system and are also a state-licensed, Medicare-certified and JCAHO-accredited, non-profit home care agency.
