About Us

ProHealth Home Care is one of Wisconsin's most comprehensive sources of in-home nursing care, in-home hospice care and in-patient hospice care.  We are part of the ProHealth Care system and are also a state licensed, Medicare certified and JCAHO accredited non-profit home care agency.

Who Do We Help?

We are a group of professionals, both clinical and non-clinical, whose mission is to serve the home health and hospice needs of Waukesha County and beyond. 

Our ProHealth Home Care team includes doctors, nurses, physical and occupational therapists, social workers, chaplains, home health aids, volunteers and many others who work closely with patients and families to coordinate a continuum of care based on the needs of each individual we serve.

We Are Different.

ProHealth Home Care is different than other home health and hospice organizations in ways both large and small. As part of the ProHealth Care System we have the strength and resources of one of the nation's most advanced community health care systems.  We are just as proud to be a trusted member of hometowns throughout Waukesha County.  ProHealth Home Hospice recently celebrated 20 years of taking care of people in their homes and AngelsGrace Hospice has served nearly 1800 patients and patient families since it opened in 2003.

Mission

ProHealth Home Hospice provides extraordinary care to patients in their homes, emphasizing quality of remaining life while alleviating pain and other symptoms.

AngelsGrace Hospice provides extraordinary care to patients of all ages in our facility, emphasizing quality of remaining life while alleviating pain and other symptoms.
