Event sponsorship application

Ask ProHealth Care in Waukesha County, Wisconsin, to sponsor your community health event.

Start by reviewing our guidelines for event and program sponsorship.

Then, complete and submit the form below at least three months before your event. Send supporting documents, such as your annual report, to community.support@phci.org. Expect to hear from us within four weeks.
