Billing and financial assistance
Learn more about billing, insurance plans accepted or financial assistance, online. View your last statement and balance, make a payment or set up a payment plan online, using ProHealth Care MyChart. If you have your guarantor or visit number, you can also pay online as a guest. For concerns related to billing or financial assistance, call us at 866-432-7855.

Medical Records Request
You can request copies of your medical records online when you visit the Ciox website. You will need to have a photo ID and mobile phone with Chrome, Firefox or Safari as the recommended browser.
The process takes 10 minutes or less to complete. You will be asked for your contact information, record types and if you want to securely access your files online or have them mailed to you. You will also need to take a photo of your driver's license or other government-issued photo ID using your mobile device or upload an image if using a desktop computer.
Before your request is submitted, you will be asked to review the Authorization to Release Protected Health Information form the software creates for you and edit or sign it.
