This was a follow-up to a procedure done a couple of months prior. I felt this could have been handled by teleconference since I didn't need an exam. Provider didn't know before appt. that exam was not needed, but could have been determined with questions asked before appointment.

Report that provides details of the visit needs to identify medication prescribed or recommended more clearly and missed 1 med completely and never took it. Don't recall what it was for, etc.
