Participation Requirements

All Students - clinical and non-clinical

Prior to participating in a placement experience with ProHealth Care, including placement in non-patient care environments (e.g. medical records, IT or lab), the following steps must be completed.

Step 1

Students need to verify with their school's coordinator that their school has completed ProHealth Care's Student Affiliation Agreement. If a school doesn't have an agreement on file, the school's coordinator should contact StudentAffiliations@phci.org to begin the process.

Step 2

Each school's coordinator makes placement requests for students through ProHealth's Student Portal. Coordinators gain access to the portal by contacting StudentAffiliations@phci.org(Only school coordinators have access to the portal - students do not have access.)

Step 3

ProHealth will contact the school's coordinator regarding the status of their student's placement request.

Step 4

If the placement request is approved, students need to complete the items below and submit verification to their school's coordinator.

Section A - Health requirements

  1. Students are required to have a MMR (Measles, Mumps and Rubella) vaccination. Or, a positive titer, two MMRs or a physician-documented disease.
     
  2. Students are required to have a Varicella (Chicken Pox) vaccination.
     
  3. Students are required to have a seasonal flu vaccination from Dec. 1 through March 31.
     
  4. Students are required to have a negative Mantoux or two-step TB test, annually.
     
  5. Students who have the potential to be exposed to blood or body fluids are required to have a Hepatitis B vaccination series.

Section B - Background checks
Both clinical and non-clinical students should ask their school's coordinator for instructions on completing the following.

  1. Students must successfully complete a Wisconsin Caregiver Background Check.
     
  2. Students must successfully complete a Wisconsin Criminal Background Check.
     
  3. Students must successfully complete a five-panel drug screen (blood test).

Section C - Additional paperwork

  1. Students need to read the Student Affiliations within ProHealth Care Policy
     
  2. Students need to read and sign the ProHealth Care Confidentiality Agreement for Students and Observers and return to their school's coordinator or instructor.
     
  3. Students need to watch the Student Orientation video and either print the validation score page or take a photo of it while it's on their computer screen and return to their school's coordinator or instructor.

Section D - Precepted students only

  1. Students need to print and bring the Preceptorship Student Orientation Checklist with them on their first day of clinical.
     
  2. Students need to print and give the Badge Request Form to their preceptor to complete and return to:
  • ProHealth Waukesha Memorial Hospital - Center for Learning and Innovation
  • Fax to 262-928-2092 or interoffice mail to the Center for Learning and Innovation at ProHealth Waukesha Memorial Hospital and your badge will be sent to your location

Section E - Clinical groups only

  1. Instructor will bring the Student Orientation Checklist to the first day of clinical.
     
  2. Instructor will bring the Instructor Orientation Checklist to the first day of clinical.
     
  3. Instructor will print, complete and return the Badge Request Form to:
  • ProHealth Waukesha Memorial Hospital - Center for Learning and Innovation
  • ProHealth Oconomowoc Memorial Hospital - security office
