Request a lung cancer screening

ProHealth Care’s lung cancer screening team offers a free screening consultation to ensure you meet the criteria for a low-dose CT lung cancer screening. If you are eligible to receive a screening, an order will be placed and you can schedule the CT scan at the location that is most convenient for you.

If you are age 50 to 77 and interested in a low-dose CT lung cancer screening, complete this short form. You will receive a call from a member of ProHealth Care’s lung cancer screening team within two business days. You will be asked a series of questions to ensure you meet screening criteria. If so, an order will be placed and you will be able to schedule the CT scan using MyChart or by phone.
