Our team of nurses, nursing assistants, therapists, social workers, chaplains and volunteers works together with our medical director your physician to provide a personalized plan of care.

Together, they help address the physical, emotional, social and spiritual needs of patients and loved ones through:

Starting hospice sooner matters

Many families wait until the final days or weeks of a serious illness to begin hospice care, yet hospice services are most beneficial when started earlier.

Earlier care gives the hospice team more time to understand the patient’s needs, manage symptoms and help the family prepare for changes. It also gives patients and loved ones more time to benefit from the emotional, spiritual and practical support hospice provides.

When our team helps manage symptoms and the demands of caregiving, loved ones can spend less time coordinating care and more time simply being together.

Care where you call home

Most hospice care is provided wherever a patient calls home, whether that's a private residence, assisted living community, skilled nursing facility or another residential setting. ProHealth’s home hospice team serves patients in Waukesha County and surrounding areas, including portions of Dodge, Jefferson, Milwaukee, Walworth and Washington counties.*

Patients do not have to be confined to bed or unable to leave home to receive home hospice care.

If additional support is ever needed, we're here for that, too.

If pain or symptoms become difficult to manage at home, inpatient hospice care at ProHealth AngelsGrace Hospice may be available to provide around-the-clock care. Inpatient services are also available at ProHealth’s hospitals in Mukwonago, Oconomowoc and Waukesha.

Our interdisciplinary team works closely with the patient’s physician and loved ones to ensure care in any setting reflects personal goals and preferences every step of the way.
