ProHealth Care, Inc. and its affiliated entities, including ProHealth Waukesha Memorial Hospital, ProHealth Oconomowoc Memorial Hospital, ProHealth Care Home Care (including hospice services), ProHealth Medical Group clinics and Waukesha Health System (collectively “ProHealth Care”, “we”, or “our”) are committed to safeguarding the information we collect from you and your representatives (“you” or “your”).

- The content of electronic forms you submit via the Website, requesting us to provide information, or to share compliments, concerns, or your story.
We also receive and store certain types of information automatically when you interact with us online or with the Websites. For example, we use:

ProHealth Care and/or our Service Providers collects and stores information that you give us directly through our Web Properties. For example, when you:
- Provide information through your MyChart patient portal;
- Send us a message through our Web Properties;
- Request patient records;
- Subscribe to our email notifications service;
- Pay your bill online;
- Make an online appointment;
- Schedule and/or use a virtual appointment;
- Perform an online check-in;
- Use site-based search functions;
- Complete an online survey.
ProHealth Care may use and process information about you, including personal information, for one or more of the purposes described in this Privacy Policy. For example, ProHealth Care and/or our Service Providers may use information about you to:
- Respond to and manage requests regarding your patient care, including scheduling appointments, processing of payment or bills, billing to your insurance plan, processing patient records, reporting test results, messaging providers, sharing clinic notes and after-visit summaries, prescription refills, and virtual visits;

- Register, maintain, and provide notifications to you about your MyChart account;
- Fulfill or meet the reason your information was collected or provided
- Present, optimize, and maintain the security of our Web Properties and their contents, which includes using reCAPTCHA to protect our Website from automated software applications (“bots”);
- Detect and prevent fraud or other crimes;
- Respond to law enforcement requests and as required by applicable law or court order;

Sharing information with third-parties
ProHealth Care may share or disclose information as provided:
- Within the ProHealth Care family: We may use and share your personal information to run our practice, improve your care, and contact you when necessary.
- Trusted Service Providers: We use certain trusted third-party service providers (“Service Providers”) to help us perform, improve, protect, and promote services on our behalf. This may include processing payments, sending you email or postal mail, providing customer support, responding to web communications, including our online chatbot, providing marketing assessments, collecting donations, event registration, providing analytics information, data hosting for the website, providing cloud-based storage and processing services, giving you access to your medical records (through the MyChart patient portal), scheduling online appointments, accessing and applying for career opportunities, implementing your privacy preferences, providing reCAPTCHA services to protect our website from bots, and other electronic services. These Service Providers only have access to information needed to perform tasks on ProHealth’s behalf in compliance with this Privacy Policy, and they are required to maintain the confidentiality of your information. If any Service Provider needs to access your PHI in order to perform services on behalf of ProHealth, they are Business Associates of ProHealth and have written agreements with us that specifically indicate that they will protect your information in accordance with this Privacy Policy and any applicable state and/or federal data protection laws. - Business transfers: If ProHealth Care purchases other businesses or their assets, sell our business assets, or are involved in a bankruptcy, merger, acquisition, reorganization or sale of assets, your information, including any category of personal data collected by ProHealth Care, may be among assets sold or transferred and may be subsequently used by a third-party.
- Lawful requests and safety: ProHealth Care Websites will disclose your personal information, without notice, only if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law or comply with legal process served on ProHealth Care or the site; (b) protect and defend the rights or property of ProHealth Care; (c) act under exigent circumstances to protect the personal safety of users of ProHealth Care, or the public; and (d) meet our obligations under state or federal law including the Department of Health and Human Services.

ProHealth’s patient portal is managed by a third-party Service Provider and does not utilize online tracking technologies, except for the use of necessary cookies required for the application to function.
Occasionally, online visitors may be asked to fill out a survey. A third party, hired by ProHealth Care, might also collect deidentified information about visits to our website. This is primarily accomplished through the use of a technology device, commonly referred to as a web beacon or action tag. The information is used only to make decisions about services or information offered by ProHealth Care.
Communications with you
All emails sent to ProHealth Care via our website are "blind" — ProHealth Care staff are not informed of the email address from which a message originates. You will not be added to our mailing list unless you specifically ask to be.
